ACO REACH Model: What is Required and How Payers, Providers Can Leverage Actionable Interoperability to Succeed

CMS’ Global and Professional Direct Contracting Model is getting a new name and look. Effective Jan. 1, 2023, the agency is replacing the GPDC model with the redesigned ACO REACH Model, intended to improve care coordination and outcomes for Medicare patients, especially those in underserved areas. The webinar outlined the incentives and requirements laid out by the new ACO REACH model and shared how organizations can leverage actionable interoperability to make them achievable. By encouraging healthcare providers and giving them tools to make it happen, unified care coordination can be realized.
