ACO REACH Model: What is Required and How Payers, Providers Can Leverage Actionable Interoperability to Succeed
CMS’ Global and Professional Direct Contracting Model is getting a new name and look. Effective Jan. 1, 2023, the agency is replacing the GPDC model with the redesigned ACO REACH Model, intended to improve care coordination and outcomes for Medicare patients, especially those in underserved areas.
