Perri Smith

Perri Smith, MBA, is Director of Population Health at Oak Street Health, a national network of value-based primary care clinics for adults on Medicare. Perri is responsible for designing and implementing strategic initiatives related to transitions of care. Prior to joining Oak Street Health, she served as special assistant to the COO/EVP at Boston Medical Center and as a management consultant with Accenture’s Federal Services Practice, where she worked on interoperability and digital health projects. Perri is deeply passionate about improving access to health care and eliminating disparities in health outcomes.
