Jennifer Houlihan

Jennifer is the Vice President of Value Based Care and Population Health for Atrium Health Wake Forest Baptist Health (now part of Advocate Health) focused on driving the transition from fee-for-service to value-based care, with a particular focus on provider engagement, population health management, and successful implementation of accountable care payment models. Jennifer also holds an appointment in the Wake Forest School of Medicine Center for Healthcare Innovation to support academic learning health system -population health implementation practice across the healthcare delivery system. Jennifer earned a Master’s in Planning from Florida State University with an emphasis in Health Policy and a Master’s Certificate in Population Health from Thomas Jefferson University.
